Appointment Information

Complimentary Phone ConsultatioN

  • 15-minutes | Free

  • Optional: Schedule this appointment if you have questions before you are ready to schedule a Nutrition Consultation. No health advice will be given during this time and there is no pressure to schedule.

Individual Nutrition Consultation

  • 90-minutes | First Appointment Only | $240 (reflects private pay discount)

  • Fill out a comprehensive health history and other intake forms online prior to your appointment.

  • During this appointment Amber will review your history and goals and get to know you better.

  • You will leave with a clear goal and action plan to start your nutrition journey.

Follow-Up Appointments

  • 45-minutes | $115 (reflects private pay discount)

  • Follow-up Visits are scheduled based on your personal nutrition plan and need for structure and accountability but the average is every 2-4 weeks.

  • 4-6 months of regular appointments is the minimum suggested time to work together.

Monthly Membership

  • 30-minutes | $85/month (4 month minimum)

  • Monthly Membership is perfect for those who want an affordable option for regular support. It includes a monthly 30-minute follow-up appointment and unlimited messaging support. Appointments can be done in-person or virtually.

  • This is available to anyone who has completed an Initial Nutrition Consultation appointment.

  • Payments will be automatically processed on the same date each month.

    Disclaimer: Membership must be maintained for at least 4 months to avoid a $50 early termination fee. After that, cancel anytime. Appointments will rollover during the membership period but any remaining appointments are forfeited at the time of termination.

Payment Information

Payment is due at or before the time of service. Cash, check, all major credit cards, as well as HSA and flex spending accounts are accepted (please confirm your plan covers licensed nutritionists prior to your appointment). 

For reimbursement from out-of-network insurance providers we can provide you with a Superbill that you can submit to your insurance for possible compensation.
